What is Family unit Connects Racine County?

Family Connects is a customs-wide nurse dwelling house visiting program for all parents of newborns in Racine Canton. Our mission is to enhance family unit and kid well-beingness.

We believe that every family has needs around the birth of a kid. The needs are different in unlike families, simply nobody is fully prepared for a baby. Family Connects provides between one and 3 nurse home visits to every family unit with a newborn commencement at about three weeks of age, regardless of income or demographics. Nurse visitors offer supportive guidance, respond to family unit questions almost newborn care, and help span the gap between parent needs and community resources. Family Foundations Home Visiting

An evidence-based home visitation program that uses the Healthy Families America model and Growing Swell Kids curriculum. Family Foundations Home Visiting is funded by WI Department of Children and Families and is a partnership between Racine County Human Services Department,Racine County Public Health Sectionalisation, and a consortium of Racine County habitation visiting providers. Plan goals include just are not limited to improving nativity outcomes; promoting kid prophylactic, health and development; improving maternal health; and, improving service coordination with and referral to other community resource and supports. What is Racine County Home Visiting Network? (RCHVN)

RCHVN logo

A group of local agencies who provide free dwelling house visitation services to pregnant and parenting families- including each mentioned above.  It is a community project to promote good for you babies and families.  Partners include Racine County Human Services Section, United Way of Racine Canton, Racine County Public Health Division, Ascension—All Saints, Professional Services Grouping, City of Racine, Wisconsin Department of Children and Families (DCF), and State of Wisconsin Department of Health Services (DHS).  Racine County Public Wellness Division is part of the Greater Racine Collaborative for Good for you Birth Outcomes (LIHF) and Family unit Preservation West.

Dwelling house Visting Staff

All program services are provided past Public Health Nurses/Educators with experience in perinatal care, kid rearing, growth and development, HFA model, Growing Peachy Kids curriculum, prevention education and community resources.
